Fill in all the information in the form below and submit to make your reservation for the next luncheon meeting.  Please select the luncheon date.  Enter your name, address, telephone number and e-mail address.  If any of these fields is left blank, your reservation will not be taken.  You will receive a call confirming your reservation.  Please submit your reservation by the Thursday before the meeting.

Please make the check payable to: HWC.  Luncheon price is $21.00, all inclusive (soup or salad, entrée, dessert, tea or coffee, tax and gratuity).

Cancellation Policy:  Anyone cancelling a reservation on-line must do so no later than the Friday before the  meeting date by calling Martha Halcrow at 407- 328-9972.  If the reservation is not cancelled by that date and you are unable to attend, you will be billed the full cost of the luncheon.
